Our Commitment to Your Privacy

Longitude Life Clinics is required by law to maintain the privacy and security of your Protected Health Information (PHI), provide you with this Notice of our legal duties and privacy practices regarding your PHI, and notify you in the event of a breach of your unsecured PHI. You may revoke your authorization in writing at any time. Revocation will not affect any actions we took in reliance on your authorization before we received your revocation.

You have the following rights with respect to your PHI:

Right to Access. You have the right to inspect and obtain a copy of your health information maintained by our practice. Requests must be submitted in writing. We may charge a reasonable fee for copying and mailing costs.

Right to Amend. You have the right to request an amendment to your health information if you believe it is incorrect or incomplete. Requests must be submitted in writing with a reason for the amendment. We may deny your request under certain circumstances and will notify you of the reason.

Right to an Accounting of Disclosures. You have the right to request a list of certain disclosures we have made of your health information. This accounting will not include disclosures made for treatment, payment, healthcare operations, or disclosures you authorized in writing.

Right to Request Restrictions. You have the right to request restrictions on how we use or disclose your health information for treatment, payment, or healthcare operations. We are not required to agree to your request, except that we must comply with a request to restrict disclosures to a health plan for services you paid for entirely out of pocket.

Right to Request Confidential Communications. You have the right to request that we communicate with you about health matters through a particular method or at a specific location. For example, you may request that we contact you only by mail or at a specific phone number.

Right to a Paper Copy of This Notice. You have the right to obtain a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

Right to Be Notified of a Breach. You have the right to be notified if there is a breach of your unsecured PHI.

Our Duties

We are required by law to maintain the privacy and security of your PHI. We will promptly notify you if a breach occurs that may have compromised the privacy or security of your information. We must follow the terms of this Notice currently in effect. We reserve the right to change our privacy practices and the terms of this Notice. Any revised Notice will be effective for all PHI we maintain. A current copy will be available at our office and on our website.

How to File a Complaint
